Original Medicare includes Medicare Part B as a necessary component providing coverage for essential outpatient services and medical supplies for treating various health conditions. This includes doctor appointments, laboratory tests, diagnostic screenings, medical equipment, ambulance services, and additional outpatient care.

What Does Medicare Part B Cost?

In 2023, the federal government set the Medicare Part B monthly premium at $164.90. If your income exceeds $88,000, you may face a higher premium. Under the original Medicare Fee-for-Service plan, an annual deductible is $226. After that, you are liable for 20% of the cost of any outpatient services, including doctor visits. Most beneficiaries choose to have Social Security income withheld each month to cover their Medicare Part B premium. You will receive a straight bill if you do not receive Social Security.

Delaying Enrollment in Medicare Part B

Some individuals may receive Medicare Part A without a premium due to their work history. Still, most people must pay a monthly premium for Medicare Part B. Suppose you are covered under an employer group health plan (either your own or through your spouse). In that case, you might choose not to enroll in Medicare Part B during your initial enrollment period to avoid paying the premium.

If you delay Medicare Part B enrollment because you have current employer health coverage, you can enroll later during a Special Enrollment Period without incurring a late penalty. If you are currently employed and covered by a group plan, you can enroll in Medicare Part B anytime. After your employer coverage or employment ends, you have an eight-month window to enroll in Part B without a penalty.

Is it possible for me to delay enrolling in Medicare Part B?

Yes, if you have current employer health coverage. If you would like to know how your insurance might work with Medicare, you should speak with your health benefits administrator. If you have employer health coverage, you can delay Part B enrollment and sign up during a Special Enrollment Period without a late penalty. Remember, retiree coverage and COBRA do not qualify for a Special Enrollment Period.

What services are not covered by Part B?

Routine physical examinations, cosmetic surgery,  vision care (including glasses or contact lenses), hearing aids, long-term care, dental treatment (including dentures), alternative health services (including massage and acupuncture), and the majority of prescription medications are not covered by Part B.

When can I apply?

Initial Enrollment Period: When you turn 65, you have a seven-month window of time to sign up for Medicare. This period starts three months before birth, includes your birth month, and lasts three months after.

  • Open Enrollment Period (October 15–December 7): During this period, you can change your Part D plan or switch from Original Medicare (Parts A and B) to Part C (Medicare Advantage).

  • General Enrollment Period (January 1–March 31): This is your chance to enroll if you were not able to during your Initial Enrollment Period.

  • Special Enrollment Period: If you delay signing up for Medicare due to current employer health coverage, you have an eight-month period after your coverage or employment ends to enroll without penalties.
